Lyra Self-Pay Terms and Conditions

This document outlines the terms and conditions for members who choose to use Lyra’s products and services on a self-pay basis. A member is considered self-pay when their sessions are not covered by an employer-sponsored benefit or a Health Plan. When you use our products and services, or see our providers, you are agreeing to all of the fine print. We encourage you to read the entire terms of use carefully.

  1. Financial Responsibility
    • By using Lyra’s services on a self-pay basis, you acknowledge and agree that you are fully responsible for the entire cost of each session, including any applicable taxes or other fees. The full session cost will be displayed to you before you book your first self-pay session. You understand that this is not a cost-sharing arrangement, but rather that you will be financially responsible for the total amount of charges for each service rendered.
  2. Payment Authorization
    • By providing and keeping a valid credit or debit card on file, you authorize Lyra to charge this payment method for the full cost of each session you attend.
      • When you will be charged: Your payment method will be charged after each session is completed.
      • Declined payments: If a charge is declined, Lyra may refuse to cover future sessions until the outstanding balance is settled.
      • Payment method updates: You are responsible for keeping your payment method up to date.
  3. Missed Appointments and Cancellations
    • Lyra’s cancellation policy requires advance notice to cancel or reschedule a session. As a self-pay client, you will be responsible for a late cancellation payment if you:
      • Cancel without advance notice
      • Do not attend a scheduled session (a “no-show”).
    • You authorize Lyra to charge a fee to your payment method on file for any late cancellations or no-shows.
  4. Agreement to Terms
    • By continuing to use Lyra’s services on a self-pay basis, you agree to these Self-Pay Terms and Conditions.
  5. Updates to Terms
    • These terms may be updated by Lyra from time to time. Your continued use of the services after any such update constitutes your acceptance of the new terms.
